The 2026 standard Part B premium is $202.90 per month, and IRMAA applies above $109,000 of income for single filers or $218,000 for joint filers.

These are the official 2026 thresholds from CMS. The surcharge is per person, so a married couple can each pay it.

Other 2026 numbers worth knowing while you budget: the Part B annual deductible is $283, the Part A hospital deductible is $1,736 per benefit period, Part D plans may charge at most a $615 deductible, and your out-of-pocket costs for covered Part D drugs are capped at $2,100 for the year. High-Deductible Plan G carries a $2,950 deductible in 2026.

Frequently asked questions
- What income does IRMAA look at?
- IRMAA uses your modified adjusted gross income (MAGI) from your tax return two years back, so 2026 surcharges are based on 2024 income. MAGI is your adjusted gross income plus tax-exempt interest.
- Can I appeal an IRMAA surcharge?
- Yes, if your income has dropped because of a life-changing event such as retirement, divorce, or the death of a spouse. You file Form SSA-44 with proof of the event. Appeals based only on a one-time capital gain generally do not qualify.
- Does IRMAA apply to Medicare Advantage plans?
- Yes. IRMAA applies to your Part B premium and to Part D coverage whether your drug coverage comes from a standalone plan or is built into a Medicare Advantage plan.
- Are these calculators a quote?

- Why is my first Medicare bill so large?
